To nominate a service member or veteran to receive a quilt of valor, you may print a nomination form and mail it (Bradford County, PA region only). You may also fill out an online form, but for recipients in our area we prefer the printed version that comes directly to us.
You will be expected to help arrange a time and place for the quilt presentation or to provide contact information for someone who will make the arrangements. The quilt will be presented in a ceremony by BCQOV volunteers. Please advise of any special circumstances, such as the recipient is in hospice, the recipient is currently deployed, etc.
